Coaching Consent & Terms

This document sets out how we will work together. Please read carefully and confirm consent by signing at the end (typing your name/date is fine for online sessions).
1) Service & Approach
I offer coaching-informed psychotherapy and psychotherapy-informed coaching focused on clarity, calm, and relational wellbeing. We will agree together whether coaching or therapy is the best fit for your needs.
2) Sessions
• Length: 60 minutes (standard).
• Location: secure video (Google Meet) or by arrangement, in person.
• Frequency: usually weekly/fortnightly; reviewed regularly.
3) Fees, Payments & Cancellations
• Fees are payable at booking via Stripe (payment plans must be kept up to date for sessions to continue).
• Cancellations/rescheduling: please give at least 24 hours’ notice; late cancellations/no-shows are charged in full.
• Late arrival: sessions end at the scheduled time; arrivals >15 minutes late may be treated as no-shows.
• Packages: multi-session packages must be used within 4 months of purchase unless we agree otherwise.
• Refunds: fees are non-refundable once a session is booked/started, except where required by law.
4) Scope & What I Don’t Do
I do not provide medical, legal or financial advice, prescribe medication, or offer crisis services. I cannot guarantee outcomes; results depend on your engagement and circumstances.
5) Client Responsibilities
• Attend sober and arrive on time.
• Let me know about relevant medical/mental‑health history and current medications.
• Use between‑session practices we agree on and inform me of changes to your wellbeing or risk.
6) Telehealth Consent & Safety
By booking online sessions, you consent to work via secure video. Technology can fail; if it does, I’ll call your phone to continue or reschedule unused time. At each online session you will confirm your current location, and I will hold an emergency contact. If I am concerned for your immediate safety, I may contact emergency services or your GP.

7) Confidentiality, Notes & Supervision
I keep brief clinical notes. I may discuss work in clinical supervision to ensure quality; your identity is protected. I won’t share information without consent except where there is a legal/professional duty (risk of serious harm; safeguarding children/vulnerable adults; terrorism/serious crime; court order; insurer/regulator requirements).
8) Data Protection (GDPR/UK)
• Data controller: Nyasha Mutavayi — info@nyasha.uk
• Purpose & lawful basis: to deliver services (contract) and manage the practice (legitimate interests).
• Processors: Google Workspace/Forms, Calendly, Stripe (payments), Zoom/Google Meet (video). Data may be stored outside the UK with appropriate safeguards (e.g., SCCs).
• Retention: records kept up to 7 years after our work ends (or until age 25 for work begun with a minor), then confidentially destroyed.
• Your rights: access, rectification, erasure, restriction, objection, data portability. See the full Privacy Notice (link on website).
9) Social Media & Communication
• Email/text are for scheduling/admin only; I aim to reply within 1–2 working days.
• I don’t offer coaching by email or instant message.
• I don’t accept friend/follow requests from clients during our work to protect privacy and boundaries.
10) Recording
No audio/video recording by either party unless we both give written consent beforehand.
11) Third‑Party Funding (Insurers/Employers)
If a third party funds sessions, I may need to share brief updates such as attendance, dates and high‑level progress/risk information. I’ll keep sharing minimal and discuss with you when possible.
12) Endings, Breaks & Referrals
Either of us may pause or end work at any time. I recommend a planned ending session. I may suggest signposting/referral if a different level of care is needed.
13) Complaints, Insurance & Regulator

14) Governing Law
These terms are governed by the laws of England & Wales and subject to the jurisdiction of its courts.
15) Consent & Agreement
•I have read and agree to the Fees, Cancellation & Late Arrival policy.
•I consent to online/telehealth sessions and to provide my location at each session, a backup phone number, and an emergency contact.
•I understand the scope & limitations and that no outcomes are guaranteed.
•I understand how my data is used and agree to the Privacy Notice.
